From the decision · page 6Because applicant’s initial request for a compensability evaluation was made simultaneously with the service of a claim form, applicant’s request for a compensability evaluation did not comport with section 4060. Accordingly, panel number 7584902, which issued as a result of applicant’s April 22, 2023 request, was invalid.
